Licensed Clinical Social Worker (LCSW)

Position Overview:

We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our interdisciplinary post-acute care team. The successful candidate will be responsible for providing high-quality mental health services, psychosocial support, and guidance to patients transitioning from hospital care to skilled care facilities settings or long-term care facilities. This role is pivotal in addressing the complex social, emotional, and psychological needs of patients recovering from acute illness or injury, aiming to enhance their well-being and facilitate a seamless transition to their next phase of recovery.

Key Responsibilities:

  • Collaborate with healthcare providers, patients, and their families to implement care plans that address patients' social, emotional, and environmental needs to accurately conduct billable chronic care management visits.
  • Conduct comprehensive assessments of patients' psychosocial needs, strengths, and limitations to develop individualized care plans.
  • Provide counseling and therapy sessions to patients and their families to support emotional well-being, coping strategies, and adjustment to post-acute care settings.
  • Act as an advocate for patients, assisting with the coordination of community resources, support services, and benefits to ensure a successful transition from acute care to home or long-term care environments.
  • Monitor and evaluate patients' progress, updating care plans as necessary to reflect changes in their condition or circumstances.
  • Maintain comprehensive, accurate, and timely documentation of all patient interactions, interventions, and outcomes.
  • Participate in multidisciplinary team meetings, contributing to the development of integrated care strategies and promoting holistic approaches to patient care.
  • Stay informed of the latest research and best practices in social work and post-acute care, integrating new findings into practice to enhance patient outcomes.

Qualifications:

  • Master’s degree in Social Work (MSW) from an accredited institution.
  • Valid LCSW license in the state of Colorado.
  • A minimum of 2 years’ experience in a clinical setting, with preference given to experience in acute or post-acute care.
  • Demonstrated ability to conduct thorough psychosocial assessments and develop effective care plans.
  • Excellent counseling and crisis intervention skills.
  • Strong advocacy and negotiation skills, with the ability to coordinate resources and services for patients.
  • Effective communication and interpersonal skills, with the capability to work collaboratively within an interdisciplinary team.
  • Knowledge of healthcare systems, Medicare, Medicaid, and other social services programs.
  • Compassionate and patient-centered approach to care.
  • Ability to maintain confidentiality and adhere to ethical guidelines and standards of practice.

Working Conditions:

  • This position requires flexibility in hours and locations, including the requirement to travel within the Southern Region—Specifically Colorado Springs, Pueblo, and Canon City.
  • The role involves potential visits to patients’ homes, long-term care facilities, assisted living locations, and occasional hospital settings.
  • Physical requirements include the ability to travel, navigate various settings, and occasionally lift or assist patients.

Salary:

$34-$45 hourly range. Actual hourly rate dependent on experience.
